I, the undersigned, player or legal guardian of the participant, hereby authorize activity supervisors/vehicle drivers, as my Agents, to consent to medical, surgical or dental examination and/or treatment. Waiver of Liability and Disclaimer.

I, the above named individual, acknowledge that participation in athletic events necessarily involves risk of physical injury. I further acknowledge that the programs of the Toledo Amateur Baseball League (TABL) are primarily administered by people who volunteer their time rather than paid professionals. In consideration for accepting the registration of the above named individual and permitting the voluntary participation of said individual in its program, I hereby release, discharge, and hold harmless TABL, its employees, volunteers and other representatives from any claims arising out of or relating to any physical injury that may result to said individual while participating in TABL-sponsored events, including any physical injury caused by the negligence of any official, referee or coach while performing his/her duties during any practices or games.

I hereby agree and consent to the following parameters as conditions of participation in the Toledo Amateur Baseball League:

I understand that I am committed to attending all practices and games. If my I am unable to attend, I will call 24 hours in advance.

I will observe all rules as established by the league President at all times.

I understand that fighting, physical abuse of players, umpires, or spectators, and the use of abusive or offensive language will not be tolerated by the league and violation of this rule could result in my banishment or suspension from the league and forfeiture of all fees paid.

I certify that I am, or will turn 18 years of age this calendar year.

I realize that there is no guaranteed playing time on any given team, associated with regular season and tournament play.

By signing this agreement I release the Toledo Amateur Baseball League from any liabilities or cost.

I fully agree that the terms and conditions of this agreement are binding.

The registration fee is $150.00 per player.
